Role of Toll like Receptor 2 Gene Polymorphism and Expression and Interleukin 6 Gene Expression in Diagnosis of Colorectal Cancer

Abstract
Background: The immune system lays a major role in colorectal cancer (CRC) development. Toll-like receptor 2 (TLR2), is predominantly expressed in tissues involved in immune function. We targeted to compare TLR2 receptor polymorphism and levels of TLR2 receptor expression between cancerous tissues and normal one and detect the relation between TLR2 gene polymorphism and its gene expression in cancerous tissue.Patients and Methods: This study is a case-Control Study that was conducted in the duration from May 2018 to March 2020 in Medical Biochemistry and General surgery Departments, Faculty of Medicine, Zagazig University. The study included 50 participants. They were classified into 2 groups; control group (25 apparently healthy control and cases group (25 colorectal cancer patients). The tissue samples were obtained as normal mucosa/cancer specimens by colonoscopy. TLR2 genes expression on tissue samples was performed by Real time PCR, using specific primers.Results: there was statistically significant difference (P< 0.05*) between case and control groups regarding TLR2 polymorphism with( DD&DI) had higher frequency among colorectal patients (64%) and II type more frequency in control group (80%). TLR2 expression (p<0.05*) had higher levels were detected in colorectal cancer patients 1.5 (1.05-3.5) versus 0.91 (0.71-1.32) in control group. Higher levels IL6 (p<0.001*) were detected in colorectal cancer patients 2.41 (1.31-4.27) versus 0.81 (0.38-1.51) in control group. There was statistically significant difference association between TLR2 polymorphism and TLR2 expression among colorectal patients (p<0.05) with higher level of TLR2 expression was reported in DD genotype.Conclusion: TLR2 polymorphism has influence on the incidence of colorectal cancer in Egypt and increase the TLR2 expression in tumor tissue. Also increased TLR2 expression in CRC patient with positive correlation is associated with downstream IL6 expression so it may be used as a marker for colorectal cancer progression.
